Appointments
Your scheduled appointment time has been reserved specifically for you. If you need to cancel your appointment we request 24 hours of notice. We certainly understand that unforeseen events can sometimes cause missing an appointment, but after missing your second appointment without notifying us 24 hours in advance, you may be charged an additional fee or possibly be no longer able to schedule appointments.
Insurance
Our office dedicates itself to maximizing your insurance benefits. However, because insurance policies vary greatly we can only estimate your coverage in good faith and cannot guarantee coverage due to the complexities of insurance contracts. We will confrirm your benefits with your insurance company prior to your appointment. Before we begin treatment at our office you will be given an estimate of what your insurance company will pay. Again, this is only an estimate. Insurance companies will not guarantee coverage with Willoughby Dental when we ask about your benefits. We would be glad to have an official pre-treatment estimate mailed to you at your request if you would like, but this is not common practice. Please remember that your insurance policy is a contract between you and your insurance company. Willoughby Dental is not a third party to that contract, but we are happy to file your insurance for you as an added free service. Your estimated patient portion, previously mentioned, must be paid at the time of service. We will allow the insurance company up to 45 days to render payment. After 60 days, you are responsible for the entire balance, paid-in-full. Knowledge of all benefits (amounts, deductibles, maximums, limitations, exclusions, waiting periods, etc) is entirely your responsibility as a policy holder and by asking us to file your insurance for you and carrying out treatment, you are in agreement with your specific insurance that you are aware of any and all provisions that they may have and will need to pay the full amount regardless of what your insurances agrees to pay after the treatment is complete. If you have any questions, our friendly team members are always available to answer them.
X-Rays
In order to properly diagnose treatment, x-rays are required by our team. If you had x-rays taken at another office 6-12 months prior to your appointment with us, we ask that you obtain them. If you do not bring your x-ray films with you or do not have them transferred you may have an additional cost due to your insurances inablitly to pay. Please note that our office does not call other offices for your records and existing x-rays, because they often require you, the patient, to verify the request.
